Anyone know where to order complete etched 1-off custom 5-digit smokebox numberplates beginning with 4 or 5?

No mention of number plates as such but it might be worth asking http://narrowplanet.co.uk/products . I've had work done by them and been very satisfied.

Try asking railtec transfers. They have a new process where they can print 3D transfers.

247 models do bespoke plates, don't know about 4s and 5s though...
