Looking at figure 8 of digest 62 there is one dimension missing in the bottom half section. What is the normal depth of ballast under the sleepers to the inside top of the brick arch?
I'm also wondering if early bridges and viaducts had lower parapet walls then the 4'6" shown. I think some were nearer 3'?

I would agree. The middle parapet portion of this beauty is possibly near 4' above 'deck height', but the outer portions are a lot lower.
The deck on the LSWR bridge at Barnstaple is about mid-girder height, and the 'parapet' is a one-sided handrail:
